Active Motion Management Guidelines

• Active motion management means that the patient motion must be tracked in real time during the course of the treatment with automatic or manual beam off trigger when the patient moves out of tolerance.

• Monitoring must be directly related to the motion of the tumor.

• Utilizing surface guidance for setup alone does not qualify for 77412.

• The physician must document medical necessity for motion management on a moving target/tumor such as thoracic, upper abdominal and left sided breast cases.

• Active motion management can be used for 3D or IMRT plans.

• The expected industry utilization for 77412 is 35% for all scenarios listed based on CMS’s review of historical claims data, utilization and disease-site utilization.
